Thyroid and neck

Thyroid swelling

The thyroid is a small gland low in the front of your neck. A lump within it is called a nodule; an enlarged gland is called a goitre. Most thyroid nodules are not cancer, but a new swelling should be checked.

What you may notice

A visible lump, pressure from a collar or a feeling of fullness may be the first sign. Some nodules are found on a scan done for another reason. New hoarseness, trouble swallowing or rapid growth needs prompt assessment.

What do the tests tell us?

Blood tests show how the thyroid is working. Ultrasound shows the size and appearance of a nodule. When needed, a fine needle takes a small sample of cells. A normal thyroid blood test does not rule out a problem within a nodule.

What happens after the results?

A reassuring nodule may be monitored with a follow-up plan. Medicines may treat an overactive or underactive thyroid. Surgery is considered for suspicious results, pressure symptoms or selected other problems. Care may involve both an ENT surgeon and a hormone specialist.

Preparing for your visit

Bring previous ultrasound reports, blood tests and any needle-sample results. Tell the doctor when the swelling appeared and whether it has changed. Difficulty breathing with a neck swelling needs emergency care.

A note on your care

“Finding a swelling in your neck can bring a lot of questions at once. Let’s take them one at a time and be clear about what each test can tell us.”

Common questions

Does every thyroid lump need a needle test?

No. The ultrasound appearance and size help decide whether sampling is useful.

Will I need my whole thyroid removed?

Not always. If surgery is needed, part or all of the gland may be removed depending on the diagnosis and your circumstances.

Can my blood tests be normal even with a thyroid nodule?

Yes. Blood tests measure hormone function; they do not show whether a nodule is harmless. Ultrasound and sometimes a needle sample answer a different question.

If a nodule is benign, do I still need follow-up?

Often there is a monitoring plan based on its features and size. Ask when to return and which changes should prompt an earlier appointment.
